Changes to both our Standard and Basic Options
 
  • We will no longer provide 8 over-the-counter (OTC) COVID-19 tests per member.
     
  • Prior approval is required for MRI, PET and CT scans (see pages 21, 40 and 80).
     
  • The Catastrophic Maximum for members enrolled in our MPDP program will now be $2,400 (see page 112).
     
  • Members who elect to receive electronic explanation of benefits (EOBs) will receive a $10 credit to their health account (see page 126).
